H1N1 Flu Information

Medication/Nurse alerts

Parents are urged to adjust a student's medication schedule so that medication may be given at home under parent supervision. If is becomes necessary for a student to take any form of medication at school, a signed note from the parent and the physician must accompany the medication (Medication Permission Form) and given to the nurse's office. All medication must be in a prescription or original over-the-counter container and kept in and distributed through the nurse's office. Copies of the Medication Permission Form are available upon request in the main office. If there is a change in medication, it is the responsibility of the parent to notify the nurse's office.

*Please note: The school cannot administer any medication (including over-the-counter medication) without the above permission form on file. Paperwork must also be completed for students who carry inhalers.

ILLINOIS STATE LAW REQUIREMENTS

Health Requirements for School Attendance include the following information

  • Preschool children are required to have the HIB vaccine.
  • Students in kindergarten, second and sixth grades must submit evidence of a dental examination by May 15th of the school year. Dental exams must be completed within one (1) year prior to May 15th.
  • Eye exam in kindergarten and students new to Illinois .

If your child wants to play interscholastic sports a sports physical is needed every year. Please call the office for accessible clinics.

Access DuPage

The DuPage County Health Department will NO longer be does school physicals. Parents who would have gone to the Health Department may use the DuPage Community Clinic through Access DuPage. You will need to bring some type of combined proof of household income (paycheck or assistance from relatives for example) for the last 2-3 months, proof of residency (utility bill) and a photo ID. This would apply for any children and parents who were not born in the US, and are uninsured. Access DuPage does not require citizenship. It is advisable to do this now since it takes 3-4 weeks to receive the Access DuPage card. This process needs to be renewed each year before the card expires. For more information call 682-7979 ext. 7676. Also, the DuPage community center has a walk in clinic Monday -Friday 1:00 to 3:00 pm.

 

IMPORTANT: All immunizations must be completed prior to the appointment for a physical, and the immunization part of the physical form must be filled in. They will not do the physical without current immunizations. For students already enrolled, please call the school Health Office for a physical form with immunization portion filled in and signed by the Health Office.

DuPage County Health Department Immunization Clinic will still be offering needed immunizations. Call the Immunization Clinic at 682-7979 for hours.
